He's not a rare, he always spawns. Like krawnight says, he's in the vault room. He also has an elite dog by his side (which can also drop blues, yay). Easy boss since he has fairly low hp.

He used to be rare, but he is now permanent, along with several other rare spawns in BRD (e.g., Lord Roccor, Pyromancer Loregrain). I'm not sure when the change came about, but it was before the release of Burning Crusade. When he was still a rare and didn't spawn, in his place would be three generic Dark Iron guards.
