When it says to maintain, do I spam agony as fast as I can for the 10 stack, or do I spam wait for the damage to increase and just before the attack is about to finish, spam again.

Someone more knowledgeable, feel free to correct me :)
Basically, it's a trade off between damage and efficiency. In general, you want the maximum amount of damage per mana spent, because mana is a finite resource.
Let's say that a standard application of Curse of Agony for it's full duration does 10 damage. So 2 stacks for the same duration does 20, 3 stacks does 30 and so on.
Now if mana isn't an issue, then yeah - spamming CoA to get to 10 stacks as quickly as you can means you get to maximum damage output more quickly, so you'll be able to do more damage overall.
