1. Нитроускорители.
2. a) Нитроускорители работают в 100% случаев (как внутри помещения, так и снаружи) в зонах Калимдора и Восточных королевств.
[Фейл присутствует, как и должен, в Нордсколе (в т. ч. на ОЛО) и в Запределье.]
б) На кросс бг, в инстах, в некоторых помещениях, как я понимаю, в случае фейла уходят на кд вместо подбрасывания.
3. а) Должен присуствовать фейл в зонах Калимдора и Восточных королевств.
б) Вопрос с инстами запутанный. Нитро значительно меняли, особенно в Кате, поэтому трудно сказать, как именно должно работать на нашем патче.

Упоминаются фейлы в разных инстансах WotLK, в том числе во время рейд-энкаунтеров. Похоже, что при неких условиях всё же подбрасывает (высокий потолок, либо «предполагаемая» возможность сесть на маунта).

В комментариях на Wowhead есть упоминание, что в помещениях должно срабатывать (подкидывать под потолок):

Something I've noticed but am looking for comfirmation:
Over the year? that I have off and on used Nitro Boost on my boots *more on than off*, I have yet to experience the malfunction while indoors *ie anywhere you cannot mount*. All of my malfunctions have been outdoors while questing/pvp'ing/farming and outdoor instances ZG and Oc. Has anyone had a malfunction while indoors or an indoor instance?

I did once, it was in the Silver Covenant Pavilion at Argent Tourny. I has just hearthed there and was going to use the boots because I'm impatient and boom.

The malfunction effect is sort of useful.
I was running with the speed boost from Lumber Mill in Arathi Basin towards Blacksmith (I was about to jump off with my parachute cloak), then i popped nitro boosts, but the result was, they threw me into the air, but i was still going forward at an amazing speed. So I decided to wait 2-3 seconds until I reach Blacksmith without cloak (I was really really high up in the air), then I popped my cloak. It took me right at the opposite side of LM.
Sometimes useful, sometimes not.

In Naxx it could backfire, however it'd just go on CD. No moon walking.

Then it was changed, never backfired once through Ulduar, ToC and ICC up til the nerf. Except this time the nerf caused you to fly up. I did it on trash before blood princes like a total idiot, same with my guild mate. We fly up, we bug through the ceiling, we aggro the Blood Queen trash.

Oh but it gets better - if it backfires and doesn't shoot you in the air it'll give you a debuff. Takes off a certain percentage of hp each second - that's taking off 6k+ a second for me. Combine that with dots.

Getting shot in the air or dying to a stupid debuff because I was on low hp makes me say things in such a way that makes rage guy look slightly annoyed.

It was bad enough in WG, now that it can happen in raids and bgs (and happen VERY often!) makes Nitro boosts the most pointless (and dangerous) thing a person can have in WoW. I mean each time you use it you're playing Russian Roulette, except that one "bullet" could kill 24 other people. Good game.

This tinker has the possibility of a backfire, resulting in the engineer being launched straight up into the air. In some cases, a slow-fall buff is applied to reduce the risk of death. In others, the distance launched is unnoticeable, appearing as if they simply failed to activate. In all cases, the cooldown period is started. Note: Nitro Boosts will never backfire while inside an instance.

Про Близзлайк частоту подбрасываний мы никогда не узнаем; вряд ли кто-то документировал это. Скорее всего, шанс фейла <5%.

Раскрытие парашюта не должно быть гарантированным. Про зависимость от кд плаща информации не видел, звучит сомнительно.

I would say it has about a 5% chance of failure, and a 1% chance or less of failing without a parachute.

On live, (as of 3.3.5) these still can malfunction, it's just really rare. I've used these hundreds of times, and it's only malfunctioned once. However, when it does malfunction, it functions similarly to the X-52 rocket helmet, but it doesn't give you a parachute.

If you see it happen, be ready to use your parachute, because it WILL kill you unless you can bubble, safe fall/feline grace, etc.

You can get a parachute, but its not a 100% guarantee. They have failed on me many times, most of the time I did receive a parachute.

I didn't see anyone mention this, but there is a chance that when it malfunctions, a parachute will be deployed and spare your armor durability damage.
